Scheme Introduction

• Pandit Deendayal Upadhyay Rajya Karmchari Cashless Chikitsa Yojana was launched by the Uttar Pradesh Government in 2022.
• The main objective of launching this scheme is to provide cashless and free of cost treatment facilities to government employees, retired employees, and their dependent families.
• Uttar Pradesh Government's Directorate of Medical & Health Services is the nodal department of this scheme.
• This scheme is designed to work like Central Government's Ayushman Bharat - Pradhanmantri Jan Arogya Yojana (PM-JAY)
• Pandit Deendayal Upadhyay Rajya Karmchari Cashless Chikitsa Yojana is specifically run for UP Government's serving and retired employees and their families.
• A State Health Card will be issued to all beneficiaries through which they can avail the benefit of free of cost and cashless treatment in any Government or Empanaled Private Hospitals.
• Beneficiaries can also avail medical benefits from any hospital that is part of the Jan Arogya Yojana.
• If beneficiaries choose to get their treatment done in empanaled private hopsital then the limit of per year treatment cost is Rs. 5 Lakh.
• Whereas there is no such limit in Government Hospitals.

• Serving or Retired UP Government Employees can apply for a State Health Card from the Official Website of the Pandit Deendayal Upadhyay Rajya Karmchari Cashless Chikitsa Yojana.
• Those who avail benefits under the Ayushman Yojana, health cards will be issued to them after deactivating their Ayushman cards.
• For this, applicants need to send a photocopy of the card along with the Ayushman Card Deactivation Request Form to upsects@gmail.com.
• Beneficiary can visit with their State Health Card in any Government or Private Hospital and avail the benefit of cashless and free of cost treatment as promised by Government of UP under Pandit Deendayal Upadhyay Rajya Karmchari Cashless Chikitsa Yojana.

Scheme Benefits

• The following benefits will be provided to eligible serving/ retired UP Government employees and their families under the Pandit Deendayal Upadhyay Rajya Karmchari Cashless Chikitsa Yojana :-
  • Free of Cost Health Care Facilities.
  • Government Employees and their dependents will have access to cashless medical services.
  • Retired UP Government Employees are also Eligible to avail free medical facilities.
  • Beneficiaries can avail benefits in any Government or Empaneled Private Hospitals.
  • There is no limit for treatment cost in Government Hospitals.
  • Whereas, Beneficiaries will receive cashless treatment in private hospitals up to Rs. 5 lakh per year.

Eligibility Requirements

• State Health under Pandit Deendayal Upadhyay Rajya Karmchari Cashless Chikitsa Yojana will be issued only for those beneficiaries who fulfil the following eligibility conditions :-
  • All UP Government Employees and their Families.
  • All UP Government Pension Holders and their Dependents are also eligible.
  • Additionally, Family and Family Pension Beneficiaries are covered.

Required Documents

• The below mentioned documents will be required at the time of applying for State Health Card under Pandit Deendayal Upadhyay Rajya Karmchari Cashless Chikitsa Yojana through online application :-
  • Aadhaar Card of Applicant.
  • Mobile Number.
  • Aadhar Card of Dependents.
  • Photograph of the Applicant and his Dependents (20 KB).
  • Dependent's Disability Certificate. (If Applicable)
  • Birth Certificate of a Child above One Year of Age.

• Fill the asked details in the application form.
• Enter details of the dependent.
• Upload all the required documents.
• Check the filled details thorougly and submit the application form.
