Application for Non approval of up Rajya Karam Chari pandit Deendayal Upadhyay cashless health card

TO The T.O Agra I ,Ashok Kumar yadav retired on 30/11/2017 from office of SP railway Agra ,have applied for up Rajya Karam Chari pandit Deendayal Upadhyay cashless health card .On Seaching i found that my card is pending due to non approval by my DDO T.O Agra .I requested pls approve my card as early as Possible.If there is any mistake in my card application pls inform me on my email ashokkumaryadav84451@gmail.com
