Chief Minister Comprehensive Health Insurance Scheme

Introduction

• Chief Minister Comprehensive Health Insurance Scheme Launched on 23.07.2009 as “kalaingar Kaappittu Thittam” by Govt. of Tamil Nadu
• Chief Minister Comprehensive Health Insurance Scheme is a part of Ayushman Bharat Pradhan Mantri Scheme (AB-PMJAY)
• It is a great initiative by government of Tamil Nadu in the making of Atmanirbhar Bharat  
• Scheme targets people having less than annual income of less than 1,20,000 per annum
• Around 1.37 CR families are covered under this scheme
• Orphans are also enrolled under this scheme
• Provide free treatment in empanelled hospitals
• As of now 1137 hospitals and 828 diagnostics centres are emplanelled under this scheme

Eligibility

• Resident of Tamil Nadu
• Name should be displayed in Family Card/Ration card
• Family income less than 1,20,000 per annum

Beneficiaries

• Spouse (wife/husband) of eligible person by law.
• Children of eligible person (till they get married, employed or attain age of 25)
• Dependant parents of eligible person

Documents Required

• Family card/Ration card
• Income Certificate by VAO(Village administrative Officer)/Revenue authorities
• Adhaar card of the beneficiaries
• Self Declaration by the head of the family

How To Apply

• Fill in the Application Form for Chief Minister Comprehensive Health Insurance Scheme
• Visit Verification Center with Required Documents (Refer Documents Required)
• Biometrics (Eye & Fingerprint Scanning) and Photo verification at verification centre
• Approval from the concerned authority and instant Generation of E-card
