Form is rejected

Dear Sir/Madam, I submitted form for mukhyamantri ladki bahin form in center located in our area (Khadegolwali) but after some days I received message that form is rejected. Then to edit the form I visit the center but they didn't found my form it's been more than 2 weeks. I am daily visit the center but they are unable to find my form and I cannot submit the another form because it's already been submitted. Please help us how can I edit the form to get approved I tried all the ways which is possible but it didn't work so please help me. Thanks & Regards
