Uttar Pradesh Laxman and Rani Laxmi Bai Award

Introduction

• Since last some years, the Uttar Pradesh Government has been announcing schemes to cater to every citizen/category of the state.
• In view to of this, the government has introduced the "Uttar Pradesh Laxman and Rani Laxmi Bai Award" scheme to honor state sports personalities.
• Under the scheme, the government will honor state sports personalites who represent their team at the state or national level.
• Additionally, those who have participated or are going to participate in events such as Olympics, Commonwealth Games, SAF Games etc are also eligible for the scheme.
• Under the UP Laxman and Rani Laxmi Bai Award, the government will honor sport personalities with a citation and Bronze statue of Laxman/Rani Laxmi Bai.
• Furthermore, the government will provide a cash reward of 3,11,000 along with the honor.
• Under the scheme, the applicants will be rewarded only once in their lifetime and it can also be awarded posthumously.
• Only such applicants will be awarded who may fulfill the predefined eligibility criteria, notified in the notification.
• Those who meet the mentioned eligibility conditions are eligible to submit their application offline.
• Such award will be distributed under three categories namely General, Veteran and Disability.
• To be eligible for the reward, the applicant must have represent their team for three consecutive years and also won a medal.
• Meanwhile, the applicants of veteran categories must not have exceed the age limit of 40 years.

Benefits of Scheme

• Under the Uttar Pradesh Laxman and Rani Laxmi Bai award the beneficiary will get following benefits: -
  • Citation and bronze statue of Laxman or Rani Bai with a scroll.
  • Will also receive a cash reward of 3,11,000.
  • The benefit of the scheme is given to following 31 sports category.

Eligibility Criteria

• Only such sports persons are eligible for the UP Laxman and Rani Laxmi Bai Award, who may fulfills the below mentioned categories: -
  • The player must be a bonafide citizen of Uttar Pradesh.
  • He/She must be a member of the sports team for atleast 3 consecutive years.
  • Those who are the member of the senior team for one year and must have participated in national/International events are also eligible.
  • Sports person must secure medal for the year he/she is recommended.
  • He/She must be participated in national competition.
  • His/Her sports should be recommended best for the year of recommendation.
  • Those who have participated in national senior championship and going to participate in Olympics World Championship, Asian Games, Commonwealth Games, Asian and Commonwealth Championship and SAF Games will also be considered eligible.
  • The maximum age of the applicant for veteran category is 40 years.
  • However, in the year of application under veteran category, individuals aged 40 or more are also eligible.
  • Government may repeal the name of the candidate from the list, in this case he/she has to returned the citation and cash prize if awarded.

• Below mentioned documents are required, to submit applications for UP Laxman and Rani Laxmi Bai Award:-
  • Sport Competition Certificate (State/National/International).
  • Identity Proof (Aadhaar Card, Voter ID, Driving License, Passport etc).
  • PAN Card.
  • Domicile Certificate.
  • Birth Certificate.
  • Passport size photograph.
  • Bank Details.
  • Affidavit (He/She is not found guilty in dope test or in any sexual harrasment case)
  • Disability Certificate (If any)

How to apply

• Eligible applicants may submit their Uttar Pradesh Laxman and Rani Laxmi Bai Award Offline.
• To submit the appliation offline, they can download application form from Department of Sport, Uttar Pradesh Website.
• In the application form, the applicants have to fill some of the details like:-
  • Applicants name.
  • Applicants Fathers Name.
  • Birth Date.
  • Profession.
  • Permanent and Postal Address.
  • Mobile Number.
  • Bank Details.
  • National/International Sports Achievement details.
• Attach all documents with the duly filled in application form.
• Submit the application form to the department, whose details are not provided in the notification.
• For such details the applicants may contact the sports authority at their given numbers.
