Form rejected

I filled my form through naari shakti doot app on my phone I filled everything correctly and submitted it and then when I saw the updates it said that my form is in pending then it got approved and accepted but later all of sudden it said it got disapproved and got rejected I checked everything again and submitted it again but I don't know what's the issue
